  • Under Minnesota guidelines, RSC-TCM services are generally available during the last 180 consecutive days of an eligible institutional stay prior to community transition.

  • No. Minnesota guidelines state that individuals cannot receive RSC-TCM and another type of targeted case management during the same month while residing in an institution.
